I am an in-network provider for Blue Cross Blue Shield and am glad to bill your insurance directly. I will also provide you with an invoice to submit to your insurance company for out of network reimbursement.
I ask that you cancel your appointment with at least 72 hours notice. Unfortunately, due to my inability to utilize that time, I will charge the full cost of the session and I am unable to bill insurance for missed appointments.
